Health Awareness Program
The LGC HealthTrust Health Awareness Program reimburses 100 percent of the cost of many health and safety related educational seminars and classes. As a GET HEALTHY Personal Health Analysis (PHA) Questionnaire participant, you will be eligible to benefit from the Health Awareness Program which offers up to $300 in annual reimbursements for every LGC HealthTrust medically covered member* engaging in eligible healthy activities like yoga, karate, kick boxing and Weight Watchers®.

Health Awareness Reimbursement Request Form
Here's what's eligible for Health Awareness reimbursement:
- Fitness facility (including Curves®) memberships
- Reimbursable for LGC HealthTrust subscribers, plus covered spouses and dependents 18 years and over
- Submission of attendance printout sheet verifying minimum attendance of 8 dates per calendar month is required for reimbursement
- You may qualify for additional discounts by showing your health plan ID card (check with your selected facility).
- One-on-one classes or counseling for Nutrition or Personal Training
- Over-the-counter tobacco cessation products when used while attending a smoking cessation class, which is also reimbursable.
*Medically covered children are ineligible for PHA participation but qualify for Health Awareness program reimbursements (except for fitness facility memberships, one-on-one classes or counseling and over-the-counter tobacco cessation products) if the parent considered “primary” on their medical coverage completes a PHA.

Health Awareness List of Approved Programs
Before you enroll in a health-related class or fitness facility, be sure to verify it is eligible for Health Awareness Program reimbursement. You can do that by reviewing our Health Awareness List of Approved Programs or calling LGC’s Member Services Department at 800.527.5001. Once you’ve confirmed that the program is eligible:
- Simply sign up and pay the course fee up front.
- Remember to get a receipt!
- Attend at least 75 percent of the program’s sessions or classes.
- Have the program’s certified instructor sign a Health Awareness Program Reimbursement Request Form (if you are unable to download this form, please call 800.527.5001 to request one).
For reimbursement of fitness facility (including Curves®) memberships, please follow the above checklist along with these additional reminders:
- Fitness facility memberships are reimbursable for LGC HealthTrust subscribers, retirees and spouses only (children do not qualify).
- Submission of an attendance printout verifying minimum attendance of 8 dates per calendar month is required for reimbursement.
- The gym must be a qualified fitness facility.
IMPORTANT REMINDER: You must complete the GET HEALTHY Personal Health Analysis Questionnaire to be eligible for the Health Awareness Program.
