Health Awareness Program
The LGC HealthTrust Health Awareness Program reimburses 100 percent of the cost of many health and safety related educational seminars and classes. As a Get Healthy Personal Health Analysis (PHA) Questionnaire participant, you will be eligible to benefit from the Health Awareness Program, which offers up to $300 in annual reimbursements for every LGC HealthTrust medically covered member* engaging in eligible healthy activities like yoga, karate, kickboxing and Weight Watchers®.

What's Eligible for Reimbursement
Here's what's eligible for Health Awareness reimbursement in 2010:
- Fitness facility (including Curves®) memberships
- Self-reported physical activity
- One-on-one classes or counseling for nutrition or personal training
- Over-the-counter tobacco cessation products (when used while attending a smoking cessation class, which is also reimbursable).
*Medically covered children are ineligible for PHA participation but qualify for Health Awareness program reimbursements for healthy activities like sports programs, dance or martial arts classes (except for fitness facility memberships, self-reported physical activity, one-on-one classes or counseling and over-the-counter tobacco cessation products) if the covered parent employed by a HealthTrust member group completes a PHA.
How to Get Reimbursed
After you've taken this year's PHA, follow the steps below for Health Awareness Program reimbursement. REMINDER: The 2010 PHA must be completed prior to March 31, 2010, to be eligible for Health Awareness Program in calendar year 2010.
For a health-related training or class:
- Before you enroll, verify the training/class is eligible for Health Awareness Program reimbursement by reviewing our Health Awareness Approved Programs or contacting LGC's Member Services Department at 800.527.5001 or memberservices@nhlgc.org. Once verified, sign up and pay the course fee.
- Get a receipt of payment for submission with your reimbursement request.
- Attend at least 75 percent of the program’s sessions or classes.
- Have the program’s certified instructor sign a Health Awareness Program Reimbursement Request Form and follow its instructions for submission.
For fitness facility (including Curves®) memberships (reimbursable for LGC HealthTrust subscribers, retirees, spouses and dependents 18 years and over):
- Submit an attendance printout verifying minimum attendance of 8 dates per calendar month.
- Get a receipt indicating membership cost for submission with your reimbursement request.
- Complete LGC’s Health Awareness Program Reimbursement Request Form.
- Next, follow the form's instructions for submission.
For self-reported physical activity (reimbursable for LGC HealthTrust subscribers, retirees, spouses and dependents 18 years and over):
- Complete the Self-Reported Physical Activity Request Form portion of LGC’s Health Awareness Program Reimbursement Request Form verifying 30 minutes of physical activity on each of 8 dates per calendar month.
- Then follow the form’s instructions for submission to receive $25 for each month you meet self-reported physical activity requirements.
- You may submit your completed Self-Reported Physical Activity Request Form annually or several times a year (e.g., quarterly).
Please allow 4-6 weeks to process your claim.
