Health Awareness Program
LGC HealthTrust offers the Health Awareness Program to support your efforts to improve and maintain your health and reward you for achieving a healthier lifestyle. In 2013, we’ve created a program that rewards you for taking the necessary steps, challenges you to pursue your goals, and supports your efforts to live a healthier life.
Health Awareness Program Eligibility
Health Awareness Program reimbursement of up to $200 per calendar year is available to enrollees, spouses and retirees who complete the onmyway™ Health Assessment (HA) and for dependent children if the enrollee completes the HA.
LGC HealthTrust medically-covered enrollees, spouses and retirees qualify for Health Awareness Program annual reimbursement of up to $100 for physical activity (including self-reported*) and up to $100 for non-physical activity classes and training after completion of the HA. Covered dependent children age 18 and older are also eligible for this annual reimbursement once the enrollee completes the HA.
Dependent Children Eligibility
Covered dependent children under age 18 qualify for up to $200 in Health Awareness Program annual reimbursement once the enrollee has completed the 2013 HA. Requests may include any combination of physical and non-physical activity.
- Reimbursement is available for eligible group classes, sports clinics or camps that have a start and end date with instruction provided. Team sports and private classes are not eligible.
- Examples of approved classes include karate, babysitting classes, CPR/first aid, and diabetes education
- Examples of non-eligible activities include AAU or sports teams or leagues, one-on-one programs, and self-reported physical activity
- Submission deadline for class reimbursement is two calendar months from the end of the class or program.
Covered dependent children age 18 and older qualify for Health Awareness Program annual reimbursement of up to $100 for physical activity (including self-reported physical activity*) and up to $100 for non-physical activity, classes, and training.
*Self-reported physical activity requires a minimum of 30 minutes of physical activity at least 12 times per month.

When Health Awareness Program Eligibility Begins
You must complete the 2013 HA to be eligible for Health Awareness Program in calendar year 2013. When you complete the HA determines your date of eligibility for Health Awareness Program, as shown in the following chart:

Eligibility begins in the quarter in which you fill out your HA. Please allow 4-6 weeks to process your reimbursement request.
Physical Activity Reimbursements
Receive up to $100 annually for a combination of self-reported physical activity (SRPA) and physical activity classes using the Health Awareness Program Request Form.
Self-Reported Physical Activity (SRPA)
Reimbursable for LGC HealthTrust enrollees, retirees, spouses and dependents 18 years and older:
- Participate in various activities that get you up and moving such as hiking, bicycling, or working out at the gym. Track your activity on the Self-Reported Physical Activity Form and be rewarded for your efforts.
- 30 minutes of physical activity on a minimum of 12 calendar days per month
- SRPA participants may receive up to $25 per quarter
- SRPA Request Form must be submitted four times per year and the submission deadline is two calendar months from the end of March, June, September and December
Physical Activity Classes
- Before you enroll, verify that the training or class is eligible for Health Awareness Program by contacting LGC HealthTrust’s Enrollee Services Department at 800.527.5001.
- Once verified, sign up for eligible program and pay the course fee.
- Obtain receipt of payment and a program description for submission with your reimbursement request.
- Participant must attend at least 75 percent of the program.
- Submit the Class/Training Reimbursement Request Form (signed by instructor), along with a program description (brochure or printed off website) and payment receipt.
- Class reimbursement submission deadline is two calendar months from the end of the program.
Examples of eligible physical activity include aerobics, boot camp, kick boxing, self-reported physical activity, swimming and Zumba®. Examples of non-eligible activity are bowling league, rock climbing, sports team, and mini-golf.
Non-Physical Activity Classes/Training Reimbursement
Receive up to $100 annually for non-physical activity classes using the Health Awareness Program Request Form. This reimbursement opportunity can be used to address nutrition, stress management, tobacco cessation and other health/safety related topics. Combination programs that incorporate physical activity with nutrition or stress management fall into this category.
For Classes
- Before you enroll, verify that the training or class is eligible for Health Awareness Program by contacting LGC HealthTrust’s Enrollee Services Department at 800.527.5001.
- Once verified, sign up for eligible program and pay the course fee.
- Obtain receipt of payment and a program description for submission with your reimbursement request. Participant must attend at least 75 percent of the program.

Examples of eligible activities include CPR/first aid, defensive driving, healthy cooking, stress management, parenting and childbirth classes and Weight Watchers®. Programs that combine physical activity with nutrition or stress management such as Create Your Weight and yoga are also eligible.
