Health Awareness Program

image of a familyThe LGC HealthTrust Health Awareness Program reimburses 100 percent of the cost of many health and safety related educational seminars and classes. As a Get Healthy Personal Health Analysis (PHA) Questionnaire participant, you will be eligible to benefit from the Health Awareness Program, which offers up to $300 in annual reimbursements for every LGC HealthTrust medically covered member* engaging in eligible healthy activities like yoga, karate, kickboxing and Weight Watchers®.

What's Eligible for Reimbursement

Here's what's eligible for Health Awareness reimbursement in 2010:

*Medically covered children are ineligible for PHA participation but qualify for Health Awareness program reimbursements for healthy activities like sports programs, dance or martial arts classes (except for fitness facility memberships, self-reported physical activity, one-on-one classes or counseling and over-the-counter tobacco cessation products) if the covered parent employed by a HealthTrust member group completes a PHA.

How to Get Reimbursed

After you've taken this year's PHA, follow the steps below for Health Awareness Program reimbursement. REMINDER: The 2010 PHA must be completed prior to March 31, 2010, to be eligible for Health Awareness Program in calendar year 2010.

For a health-related training or class:
