You can also email us. We strive to respond to your email within one business day.

Should I mail LGC HealthTrust the paperwork for adding a member onto a COBRA policy before or after I receive the enrollee's first COBRA payment?
You should wait until you receive an enrollee's first COBRA payment before submitting LGC HealthTrust the paperwork to add or reinstate a member onto a COBRA policy. This will ensure that COBRA benefits are not utilized before payment is received. It will also prevent your group from being held liable for the initial COBRA premium due should the enrollee decide not to make that first COBRA payment.

How should an enrollee go about changing their primary care provider (PCP) if they need to do so?
Enrollees who need to change their PCP should call Anthem Blue Cross and Blue Shield directly at the service phone number listed on back of their health plan ID card.

Do I need to mail original forms to LGC HealthTrust in addition to faxing them?
No. Please do not mail original forms to LGC HealthTrust if they have been previously faxed to us. The one exception is when you are submitting life insurance applications to us. Since life insurance applications require a legal signature, originals of those forms must also be mailed to LGC HealthTrust whenever you fax them to us for processing.

What benefits are employees who have been called to active or training duty as reservists entitled to upon returning to work after uniformed service?
Upon returning to work after uniformed service, individuals are generally entitled to all benefits they had at the time service began per the Uniformed Service Employment and Reemployment Rights Act of 1994 (USERRA). It's designed to help employees and employers understand their rights and responsibilities under numerous federal employment laws.

Can our group make premium payments through Electronic Funds Transfer?
Yes. LGC HealthTrust can accommodate member premium payments by wire or Automated Clearing House (ACH) transfer. If you are already set up with your bank to transfer money from your group's account electronically, the process is simple. Just call our toll-free number in New Hampshire at 800.852.3358 and ask for LGC's accounting manager in the Finance Department or email them. They will be happy to provide you with LGC HealthTrust's routing and account numbers to initiate the electronic funds transfer (EFT) or ACH process. You will then be given a dedicated fax number and email address for advising our Finance Department of which LGC HealthTrust account number(s) your payments apply to and of your payment amounts. No more check writing to fuss with or postage charges to incur! If you would like to start using the EFT process for premium payments but are unsure if you can, please check with your bank first to ensure that you have the proper computer software installed to make EFT possible.

What needs to be done when LGC HealthTrust-covered subscribers get divorced or legally separated?
A subscriber who is either getting divorced or legally separated needs to complete LGC HealthTrust's Medical and/or Dental Application and Change Form (HT035) by checking off "Divorce/Legal Separation" and indicating the actual date of the divorce or legal separation in the "Reason for Completing Form" box. Additionally, the subscriber must complete and submit a Notice of Divorce, Legal Separation or Dissolution of Civil Union (HT008) form. Both forms are downloadable as PDFs from our Forms and Resouces web page. In some cases, it may be necessary for LGC HealthTrust to request a copy of the related court decree. Is an ex-spouse eligible for COBRA?
Yes. An ex-spouse is eligible to enroll in COBRA.

How long may an ex-spouse stay on COBRA?
An ex-spouse may stay on COBRA up to a maximum of 36 months.

If a subscriber has a two-person or family membership, can their ex-spouse remain on that membership (in lieu of paying an additional one-person membership rate)?
An ex-spouse may remain on an employee's two-person or family membership through the LGC HealthTrust member's health plan until the subscriber re-marries. The employee or their ex-spouse may, however, have premium payment obligations for the ex-spouse's coverage. Any ex-spouse who is responsible for making full or partial premium payments to the employer for such coverage must notify the employer and LGC HealthTrust, in writing, within 31 days of the court decree that continued coverage is requested.
